• Monitoring symptoms and treatment effectiveness is essential for the early identification of adverse effects or signs of relapse, allowing for swift and effective intervention. The nurse acts as a liaison between the patient and the physician, facilitating therapeutic adjustments and referrals to other professionals within the multidisciplinary team, such as physiotherapists, nutritionists, psychologists, or other medical specialists. Continuous monitoring also allows for the anticipation of consultations in cases of symptom worsening;
• Education and counselling are key aspects of the consultation. The nurse guides the patient in adopting both pharmacological and non-pharmacological strategies for symptom management, relapse prevention, and pain and fatigue control. The consultation also promotes self-care, joint protection, and balanced energy management, addressing topics such as sexuality and pregnancy;
• Emotional support and counselling are central to the follow-up process, helping patients find positive ways to cope with their condition and enhance their psychological well-being;
• Regular monitoring is crucial to encourage patients’ autonomy and confidence in managing their illness and treatment, promoting therapeutic adherence and improved health outcomes;
• Evaluation and measurement of physical symptoms such as pain, fatigue, joint stiffness, and inflammation, as well as assessment of mental and emotional health, quality of life, family support network, and self-care capacity. These are conducted using health questionnaires, quality-of-life scales, and specific disease activity assessment tools.

From an interdependent perspective, and to ensure a prompt response to unscheduled consultations, the service provides patients with an integrated action protocol that guides clinical triage based on established flowcharts. This process ensures efficient intra-hospital referral in cases of disease exacerbation.

Independently, patients followed by this service benefit from continuous and personalised follow-up through teleconsultations. These sessions serve to validate previous education, clarify doubts, monitor treatment progress, and assess health status.
Additionally, the service manages its communication channels, including the institutional email and telephone line, working closely with the medical and administrative teams to coordinate diagnostic and therapeutic procedures, manage ongoing treatments, issue reports and supporting documentation — all contributing to the optimisation of treatment processes and personalised patient care.
This project is being developed as part of a research initiative by the Autoimmune Disease Study Group (NEDAI) of the Portuguese Society of Internal Medicine (SPMI).
The registry’s main objectives include characterising the progression patterns of autoimmune diseases in Portugal, improving care quality, identifying prognostic markers, therapeutic response indicators, and risk factors for adverse effects. It also aims to integrate with European registry networks and contribute to scientific advancement in this field.
Patient participation is voluntary and formalised through the signing of an informed consent form, which is recorded in the platform. Questionnaires are either completed remotely or during the consultation, depending on the clinical context and patient preference.
The use of these tools, in combination with clinical assessments, optimises follow-up and ensures that patients receive personalised and timely care. This contributes to reduced comorbidities, fewer emergency visits, and the prevention of disease flare-ups — all central goals of the multidisciplinary treatment team.
This includes the administration of biological therapy, immunomodulators, or human monoclonal antibodies via the subcutaneous route. It also involves individualised education for self-administration at home and follow-up after administration to monitor potential adverse effects, reinforce education, and clarify any questions through teleconsultation or email contact.
